As a courtesy to you, our practice will file all insurance claims. However, we ask that you supply our office with the current and correct insurance cards at the time of service for each visit. If you come without the insurance card, full payment is expected at the time of service.

If your insurance company charges a co-pay for your office visit, the co-pay may not cover the procedures performed while in the office. Depending on your plan, procedures such as biopsies, surgeries and wart treatments may be subject to your policy’s deductible.

Due to the uncertainty in the insurance market place, it is your responsibility to ensure that we are listed as a provider in your network and your visit will be covered by your specific plan. We will do our best to assist you.
